Cal Aim Case Manager – Adult Shelter

Principal Responsibilities:

Case Management & Client Support

  • Manage a caseload of 6–8 shelter clients.
  • Conduct weekly case management meetings and document housing plans and goals.
  • Support clients with physical and mental health needs.
  • Develop and maintain individualized care plans and notes to meet program and funding compliance.
  • Provide crisis management support for both shelter and housing participants.

Program & Data Management

  • Enter and track client data in HMIS and Cal Aim systems.
  • Monitor and document program outcome measures and generate required reports.
  • Prepare program transfers, daily rosters, and other key reports.
  • Ensure data security and confidentiality.
  • Support the Senior Program Director with Cal Aim processes, including:
  1. Data collection, spreadsheets, and reporting.
  2. Billing, treatment authorization requests, and follow-ups.
  3. Running HMIS reports for accuracy and data quality.
  4. Collaborating with staff to improve data completeness.
  5. Support with monthly audits.

Teamwork & Communication

  • Actively participate in weekly program staff meetings, monthly all-staff meetings, trainings, and weekly supervision.
  • Report program, client, and facility needs promptly.
  • Maintain open, respectful, and collaborative relationships with colleagues, clients, and community partners.
  • Uphold confidentiality, boundaries, and an attitude of compassionate service.

Communication:

  • Ability to read and interpret professional materials and regulations.
  • Strong writing skills for reports, correspondence, and procedure manuals.
  • Ability to present information effectively and respond to questions from groups or individuals.

Qualities:

  • Resourceful, flexible, compassionate, and kind.
  • High cultural awareness and ability to contribute to a diverse workplace.
  • Comfort in occasionally performing physical tasks such as stooping, kneeling, or crouching.

Skills:

  • Strong time management and organizational skills; able to prioritize effectively.

Minimum Qualifications/Requirements

Education or Training Equivalent to:

  • Bachelor’s degree in social work, psychology, or related field OR at least 5 years of related residential experience.

Minimum Additional Related Experience:

  • 3+ years of experience working with under-resourced/low-income populations; direct experience with homelessness preferred.
  • Experience in residential or housing programs.
  • Strong documentation and case file management skills; experience with databases such as Salesforce, Apricot, HMIS, or Cal Aim.
  • Working knowledge of SSI/SSDI/SDI benefits.
  • Familiarity with harm reduction, mental health, life skills, money management, and basic math.
  • Proficiency with Microsoft Office Suite.
  • Spanish bilingual highly desirable.

Physical Requirements:

  • Ability to sit, and use a computer mouse, keyboard, and monitor.
  • Ability to stoop, crouch, kneel, and/or crawl on occasion.
