For 50+ years, Homeward Bound of Marin has helped individuals and families experiencing homelessness find their path to housing and stability with the goal of ending homelessness in Marin County. We operate Marin County’s only family shelter, as well as transitional and long-term supportive housing, job-training and placement, and services. Our mission is “Opening Doors to Safety, Dignity, Hope, and Independence.” We are looking for the right person to join our family program team to support Enhanced Care Management offered through CalAIM.
The Opportunity:
We are seeking a licensed clinician to provide part-time clinical consultation to staff delivering services through our CalAIM Enhanced Care Management (ECM) program. The consultant will support staff collaborating with participants experiencing housing instability, homelessness, and complex medical and behavioral health needs by offering guidance on care planning, complex cases, and documentation practices. We are seeking a clinical who can provide consultation and guidance that aligns with Housing First, trauma-informed care, and harm reduction principles. This position does not include direct client services, psychotherapy, or clinical supervision toward licensure. Program staff remain responsible for participant care coordination and documentation.
Key Responsibilities:
- Offering guidance on ECM plans and care planning approaches.
- Limited client case conferencing.
- Complete approvals of ECM care plans within designated timeline to ensure program compliance.
- Participate in periodic case consultation meetings with program staff.
- Provide recommendations related to behavioral health considerations, trauma-informed engagement, and documentation practices.
- Support staff collaborating with participants experiencing serious mental illness, substance use challenges and complex medical or social needs.
Qualifications Required:
- Active California license as an LCSW, LMFT, or LPCC
- License must be active and unrestricted
- Minimum 2 years of post-licensure clinical experience
Qualifications Preferred:
- Experience working with nonprofit or community-based programs
- Familiarity with housing and homelessness services
- Knowledge of trauma-informed care or harm reduction approaches
- Familiarity with CalAIM or Medi-Cal care coordination programs
- Knowledge and familiarity working with diverse populations and offering culturally competent interventions.
