Housing Case Manager – Adult Shelter

We’re looking for a Housing Case Manager to join our 24-hour adult shelter team and support individuals experiencing homelessness as they work to secure housing, build stability, and advance toward self-sufficiency. In this role, you will support residents, foster a safe and respectful environment, and help keep programs running smoothly while delivering short- and long-term case management, supportive services, and access to job training and placement opportunities. Through consistent, onsite support, you’ll play a key role in ensuring residents feel supported, seen, and genuinely set up for long-term success.

Education or Training Equivalent to:

Bachelor’s degree in social work, social welfare, psychology, or a related field, or at least 3 years of experience in a related residential or housing program.

Qualifications and Abilities:

  • Minimum of two (2) years’ experience working with under-resourced or low-income populations; experience working with individuals experiencing homelessness preferred.
  • Bilingual (English/Spanish) preferred
  • Experience in residential and/or housing programs.
  • Knowledge of Housing First and harm-reduction principles.
  • Working knowledge of SSI, SSDI, and SDI.
  • Experience with data entry systems (HMIS, Salesforce, or similar).
  • Proficiency with Microsoft Word, Excel, cloud-based systems, and Adobe.
  • Comfort working in a culturally and ethnically diverse environment.

Physical Requirements:

  • Ability to sit and use a computer mouse, keyboard, and monitor for moderate periods of time (1 – 3 hours per day).
  • Ability to stoop, crouch, kneel, or crawl occasionally.
  • Ability to lift or move objects up to 40 pounds.
  • Enthusiasm for working with individuals with previous experiences of homelessness.

Key Responsibilities:

  • Incorporate CalAIM care coordination and housing navigation strategies into individualized care and housing plans, in collaboration with healthcare, behavioral health, and community-based partners.
  • Provide housing-focused case management for individuals and in group settings.
  • Identify and address barriers to housing, including chronic homelessness, mental and physical health needs, substance use disorders, under-employment, lack of income, and other systemic barriers.
  • Skillfully prevent and/or resolve conflicts using a strength-based approach.
  • Respond to urgent and emergency situations, including coordination with EMTs, fire, law enforcement, psychiatric services, and community partners as needed.
  • Prevent and resolve conflicts using a strength-based, trauma-informed approach.
  • Provide supportive services, guidance, and referrals to community resources.
  • Engage program participants regularly and consistently to support program participation and progress toward housing stability.
  • Maintains awareness of the overall shelter environment and collaborates with the Program Manager to identify, report, and help address facility–related needs or concerns to support a safe, functional, and welcoming space for participants and staff.
  • Perform other duties as assigned.

Documentation and Compliance:

  • Complete timely, accurate, and compliant documentation, including case notes, assessments, care plans, and progress updates, in accordance with policies and funder requirements.
  • Ensure client records clearly demonstrate eligibility, service delivery, engagement, and progress toward housing goals.
  • Maintain accurate and timely data entry in HIPPO (Salesforce database) and HMIS.
  • Meet all documentation and case management deadlines, including intake, enrollment, reassessments, housing goals, and discharge documentation.
  • Participate in quality assurance, monitoring, and audit processes, including responding to documentation reviews and corrective actions.

Case Management, Housing-Focused Services, and Outcomes

  • Develop, implement, and regularly update individualized, client-centered housing and care plans.
  • Monitor, document, and adjust plans proactively to address changing needs or barriers.
  • Coordinate services with internal teams and external partners to ensure continuity of care and timely referrals.
  • Document housing and service outcomes to support program performance measurement and funder reporting.

Property Management and Housing Placement:

  • Manage a full caseload plus weekly meetings with strong organization, time management, and prioritization skills.
  • Communicate clearly and professionally with supervisors, team members, volunteers, and program participants.
  • Participate in weekly supervision, program meetings, all-staff meetings, and required training.
  • Maintain confidentiality and comply with all agency policies, including HIPAA where applicable.
  • Contribute positively as a collaborative, accountable member of the Homeward Bound team.

Written & Verbal Communication Skills:

  • Ability to read and interpret documents, write routine reports and correspondence, and speak effectively before small groups of clients or employees of the organization.
  • Communicate clearly and professionally with supervisors, team members, volunteers, and program participants.

Relationships:

  • Foster open and collaborative relationships with colleagues, working as a cohesive team with all Homeward Bound staff.
  • Uphold strict confidentiality regarding all program participants.

*$28 – $30 per hour DOE with a competitive benefits package which includes 100% employer paid medical, vision and dental insurance, 2.5 weeks’ vacation, and access to a 403b retirement plan.
