Program Coordinator – Supportive Housing

We’re looking for a Housing at Last (HAL) Program Coordinator to join our HUD-funded housing team and support individuals integrating into the community after experiencing chronic homelessness. In this role, you’ll work under the direction of the HUD Program Manager to provide resources, housing retention support, and consistent, in-person engagements to tenants living in scattered site units throughout Marin County. You’ll offer a balance of encouragement and accountability by assisting residents with upholding program agreements and rental agreements, pursuing personal goals, navigating challenges, and strengthening their stability while maintaining permanent housing.

Education or Training Equivalent to:

  • Bachelor’s degree in social work, social welfare, psychology, or a related field, or at least 3 years of experience in a related residential or housing program.

Qualifications and Abilities:

  • Minimum of three years’ experience working with under-resourced or low-income populations; experience working with individuals experiencing homelessness preferred.
  • Experience in residential and/or housing programs, including familiarity with the Housing First model.
  • Knowledge of Housing First and harm-reduction principles.
  • Working knowledge of SSI, SSDI, and SDI.
  • Working knowledge of local resources for low-income individuals.
  • Experience with data entry systems (HMIS, Salesforce, or similar).
  • Proficiency with Microsoft Word, Excel, cloud-based systems, and Adobe.
  • Comfort working in a culturally and ethnically diverse environment.
  • Working knowledge of substance dependence disorders and mental health disorders.
  • Knowledge of life skills, money management, and tenancy rights under fair housing laws.

Physical Requirements:

  • Ability to sit and use a computer mouse, keyboard, and monitor for moderate periods of time (1 – 3 hours per day).
  • Ability to stoop, crouch, kneel, or crawl occasionally.
  • Ability to lift or move objects up to 40 pounds.
  • Enthusiasm for working with individuals with previous experiences of homelessness.

Key Responsibilities:

  • Deliver services and resources that promote housing retention and support individual goals of increasing economic independence and maintaining stable housing.
  • Maintain active engagement with a caseload of 27 individuals residing in units of scattered site housing.
  • Partner with Coordinated Entry system for new housing placements.
  • Assess and document eligibility for program participation based on HUD guidelines for chronic homelessness.
  • Perform intakes when individuals are enrolled in the HAL program and enter all intake information into HMIS and other databases as needed.
  • Meet regularly and consistently with individuals to assure program participation and progress.
  • Respond to neighborhood and landlord concerns in a timely manner; proactively problem-solve with residents.
  • Work with individuals on this caseload on a one-to-one basis and in group settings during scheduled home visits and community events.
  • Address barriers to housing stability: recovery/addiction/relapse, mental health supports, under-employment, navigating shared housing and various other obstacles.
  • Manage conflicts and crisis situations as they arise.
  • Represent agency to landlords and other community partners while serving residents in housing programs.
  • Coordinate monthly home visits while providing education and support to residents in maintaining their housing.
  • When necessary, seek and identify new housing units for program participants within the community.
  • Coordinate program newsletters, announcements, and monthly participant activities.
  • Provide supportive counseling, guidance, supervision, & referrals to community resources.
  • Interact as part of a team with other Homeward Bound staff and communicate pertinent shift information both verbally and in writing.
  • Maintain daily program participant records and collect information as required by the programs.
  • Attend and participate in staff meetings, supervision, and training.
  • Provide emergency services as needed, as well as ongoing case management which may include interacting with law enforcement, psychiatric services, and other community agencies and staff.
  • Report program, resident, and facility needs to appropriate persons.
  • Document and track required program outcome measures, respond to requests for information in a timely manner and collect and enter data into data management systems such as the HMIS system.

Perform other relevant duties as assigned.

Documentation and Compliance:

  • Complete timely, accurate, and compliant documentation, including billable case notes, assessments, care plans, and progress updates, in accordance with policies and funder requirements.
  • Ensure client records clearly demonstrate eligibility, service delivery, engagement, and progress toward housing, health, and personal goals.
  • Maintain accurate and timely data entry in HIPPO (Salesforce database) and HMIS.
  • Meet all documentation and case management deadlines, including intake, enrollment, reassessments, housing goals, and discharge documentation.
  • Participate in quality assurance, monitoring, and audit processes, including responding to documentation reviews and corrective actions.

Case Management, Housing-Focused Services, and Outcomes:

  • Develop, implement, and regularly update individualized, client-centered housing and care plans.
  • Monitor, document, and adjust plans proactively to address changing needs or barriers.
  • Coordinate services with internal teams and external partners to ensure continuity of care and timely referrals.
  • Document housing and service outcomes to support program performance measurement and funder reporting.
  • Develop, implement, and monitor financial literacy services tailored to covering topics such as budgeting, credit repair, savings strategies, and employment preparedness.
  • Assist program participants in housing retention, job readiness skills, job placement and retention services, education on living skills, navigating resources and referrals, eviction diversion, and addressing physical and mental health issues.

Written & Verbal Communication Skills:

  • Ability to read and interpret documents, write routine reports and correspondence, and speak effectively before small groups of clients or employees of the organization.
  • Communicate clearly and professionally with supervisors, team members, volunteers, and program participants.

Relationships:

  • Foster open and collaborative relationships with colleagues, working as a cohesive team with all Homeward Bound staff.
  • Uphold strict confidentiality regarding all program participants.
